Immunization Check-List:
1) Arrive on time, or a maximum 15 minutes early
2) Bring identification (& your health card if you have one)
3) Bring your consent form
4) Wear a mask
5) Wear a short-sleeved shirt

Immunization Records & Card:
Find your immunization records here. You will need a MB health card to access these records online.

Fourteen days after your 2nd immunization, you are fully immunized and eligible to receive your Immunization card. If you do not have a MB health card, you are still eligible to receive an immunization card but you will need to call 1-844-626-8222 (1-844-MAN-VACC) for assistance.
